Please call our office at (402) 489–4600. Our receptionists answer the phones from 8 – 5 pm.

It is our goal at FHA to see all patients with non–urgent or routine examinations within two weeks of the request. We try to see urgent cases the same day. However, in the event this is not possible, medical staff will triage the call and work with your physician to determine the best course of action to take.

» If you have an appointment:
Please arrive 15 minutes prior to your first appointment at FHA so we can complete registration and prepare for your visit with your physician. If you are unable to keep your appointment, please call us within 23 hours so that we may use your appointment time for another patient.

Please bring the following items with you to every appointment:
  • Insurance card(s)
  • Current medications (in their original containers with pharmacy labels, if possible)
  • Driver’s License or photo ID
  • Copay – if your insurance requires one.
If this is your first appointment at FHA, please make certain all pertinent medical records have been sent to our office prior to your appointment or bring them with you. If your insurance coverage changes, you must notify our office and allow us to copy your new insurance cards.

If you are scheduled for fasting laboratory work:
  • Do not eat anything for 12 hours prior to your appointment time. You are ENCOURAGED TO DRINK WATER as this may aid in drawing your blood. DO NOT drink coffee, pop, juices, etc. Drink water only.
  • If you have high blood pressure, you should take your usual medications in the morning with water.
  • If you are diabetic, please call the office for specific instructions. When calling, ask to speak with the nursing staff for your physician.
  • If you have questions, please call (402)489-4600 and ask for the appointment desk.

New Patient Information
In an effort to maintain the most up-to-date information, we ask that every FHA patient complete our Patient Information Forms prior to being seen by a physician. These forms are filled out and updated annually to ensure that the most recent information is in the patient chart.
Medical Records
We ask that every Family Health patient complete our Medical Records Release Form prior to being seen by a physician. If you have any medical records other than those at FHA, a medical records release is needed to grant our physicians access to those records. Family Health Associates respects the privacy of your medical records and will fully comply with all applicable regulations.
